What is Mometasone Furoate?Mometasone Furoate

Mometasone furoate is a glucocorticosteroid used topically to reduce inflammation of the skin or in the airways. It is a prodrug of the free form, mometasone (INN).

What is the mechanism of action?

Unbound corticosteroids cross cell membranes and bind with high affinity to specific cytoplasmic receptors. Inflammation is decreased by diminishing the release of leukocytic acid hydrolases, prevention of macrophage accumulation at inflamed sites, interference with leukocyte adhesion to the capillary wall, reduction of capillary membrane permeability, reduction of complement components, inhibition of histamine and kinin release, and interference with the formation of scar tissue. The antiinflammatory actions of corticosteroids are thought to involve phospholipase A2 inhibitory proteins, lipocortins, which control the biosynthesis of potent mediators of inflammation such as prostaglandins and leukotrienes. Mometasone furoate has been shown in vitro to exhibit a binding affinity for the human glucocorticoid receptor which is approximately 12 times that of dexamethasone, 7 times that of triamcinolone acetonide, 5 times that of budesonide, and 1.5 times that of fluticasone.

How to dose Mometasone Furoate?

Dose of mometasone furoate administered as an oral inhalation powder is expressed as the nominal (labeled) dose contained in the Twisthaler device. The amount of drug delivered to the lungs depends on factors such as the patient’s inspiratory flow.

Each actuation of the Twisthaler inhaler contains 110 or 220 mcg of mometasone furoate inhalation powder and delivers approximately 100 or 200 mcg of mometasone furoate, respectively, per activation from the mouthpiece.

What are the side effects?

The most common side effects associated with nasal mometasone furoate are headache, nasal irritation, sneezing, and, occasionally, bleeding from the nose. Nasal septal perforation, fungal infection of the nose, and disturbances of taste and smell have been reported. Higher doses of mometasone may cause suppression of the body’s ability to make its own natural glucocorticoid in the adrenal gland. People with suppression of their adrenal glands (which can be diagnosed by a doctor) would need increased amounts of glucocorticoids, probably by the oral or intravenous route, during periods of high physical stress or acute illness when glucocorticoids are particularly important. Intranasal steroids may cause growth suppression, weaken the immune system, and may increase the risk of glaucoma, and cataracts. Allergic reactions, including swelling of face, throat and tongue, as well as rash, hives, and breathing problems may occur.

Mometasone furoate should be kept at room temperature, 4-30 C (39-86 F).
